For the fastest review and activation of your partnership, please have the following five (5) documents ready to upload before you begin the application:

  • Certificate of Insurance (COI):

    Showing required General Liability, Auto Liability, and Cargo coverage.

    Crucially: Must name Carolina Cargo Solutions, LLC as the Certificate Holder.

  • W-9 Form:

    Signed and dated for tax and payment compliance.

  • FMCSA Authority Letter:

    Your Operating Authority (MC/DOT) letter confirming active status.

  • ACH Authorization / Banking:

    A clear image or PDF of a Voided Check or an official Bank Letter to set up direct deposit.

  • Notice of Assignment (NOA):

    Only if you use a Factoring Company. Please have the NOA ready to upload to protect your financing agreement.
